Parent Forms
New Student Packet
All students are required to have their completed forms submitted to the school office before the first day school.
For new students, please print the entire packet. Individual forms are available below for returning students.
Memorandum of Understanding
Please read, sign and return to the school office.
Emergency Contacts
This form must be updated & filled out by the first day of school every year.
Universal Health
All parents must submit and update their child’s most current vaccinations (including flu) and physicals prior to the first day of school and throughout the year as needed, in order for their child to remain in school.
DCF Information & Parent Sign-Off
The State of New Jersey requires all parents to acknowledge that they have read the “NJ Department of Children and Families” sheet every year. This paperwork is required for us to remain a State Licensed School.

Allergy Alert for Classroom and Food Allergy Action Plan
If your child has ANY type of food allergy, we ask that you fill out the Allergy Alert form to post in the classroom, to advise other parents of your child’s food allergy. The Food Allergy Action Plan requires signatures from your child’s doctor as well as a parent.
